WebAIRS News - events, new features and web pages recently released
  • Reporting of "Near Misses" to webAIRS

The latest e-News from ANZCA, the ASA and the NZSA encourages the reporting of ¨Near Misses¨ to webAIRS. A ¨Near Miss¨ is defined by the World Health Organisation as ¨an error that has the potential to cause an adverse event (patient harm) but fails to do so because of chance or because it is intercepted¨.

Learning from near misses and adverse events is an important aspect of perioperative patient safety monitoring. We are encouraging the anaesthetic community to ensure that they report all near misses, as well as adverse events, so that they become aware of emerging risks and communicate these to prevent adverse events reaching the patient.

A full explanation with examples is included in the document, ¨Reporting incidents to webAIRS¨ which can be accessed using the link below. To report an incident to webAIRS please login using the menu options above.

About ANZTADC - the parent organisations that created webAIRS

ANZTADC is the Australian and New Zealand Tripartite Anaesthetic Data Committee. This committee represents and is funded by three organisations, the Australian Society of Anaesthetists, the New Zealand Society of Anaesthetists and the Australian and New Zealand College of Anaesthetists. The committee has developed 'WebAIRS' (web based anaesthetic incident reporting system). This program can be incorporated into hospital systems to assist anaesthetists to report, evaluate and receive information regarding anaesthetic incidents. The results of the incident analyses may be fed back into the system and so the 'loop' can be closed as part of a quality improvement system.
